What you notify / file

Notify: Superintendent of the district where the child resides; Initial Registration Form (first year/new district); Letter of Intent to Continue (subsequent years); MDE forms; Initial by Oct 1 or within 15 days of withdrawal; annual continuation by Oct 1 until child turns 16 (17 if started after 16).

Testing / evaluation

Nationally norm-referenced standardized achievement test (waived if accredited by a MN-recognized agency, e.g., HBEAA)
