Notify: District school superintendent of the county of residence; Written notice of intent (legal names, addresses, birthdates of students); Within 30 days of establishing the program (one-time, not annual); termination notice within 30 days of ending.

FL-certified teacher portfolio review/discussion; nationally normed test by certified teacher; state assessment test; FL-licensed psychologist evaluation; or mutually agreed tool
